Nail treatment consent form
No children or babies are allowed during any services.
This is a home-based nail salon, with pets at present. Please consider this when booking with me. I am only able to accommodate one person at a time. Please make sure you arrive without company.
🔴 Absolute Contraindications (No treatment can be carried out!)
These conditions pose a risk of infection, cross-contamination, or worsening of the issue:
-Fungal infections (e.g., onychomycosis – fungal nail)
- Bacterial infections (e.g., paronychia – inflamed skin around the nail)
- Viral infections (e.g., warts or herpes near the hands or nails)
- Severe nail or skin diseases (e.g., psoriasis in an active flare, eczema with open sores)
- Greenies (Pseudomonas infection – often seen as a green stain under acrylic/gel)
- Unexplained lumps, swelling, or discolouration
- Open wounds or cuts around the hands or nails
- Hematoma (blood under the nailplate)
Clients with warts or verrucas on their hands should not book in for nail services, as they can be highly contagious and the HPV virus can be transferred through physical contact.
"Warts and verrucas can be spread through skin-to-skin contact or contaminated surfaces with the virus. You're more likely to spread a wart or verruca if your skin is wet or damaged," Says HSE's website

If the client fails to mention such a condition (warts, verrucas present), the appointment will be cancelled and the client will lose the deposit.
I agree to wash my hands, and nails before my appointment in order to minimize the spread of viruses.
Furthermore, I confirm that I have not been diagnosed with COVID-19 last 14 days.
I verify I am not on any blood thinners or any other medication which affects nail enhancement.
I verify I have no fungal or bacterial infection on my nails. (No discolouration on nails, no thickened nails, nails are fully intact, not lifted from the nail bed, no haematoma or any active bleeding is on the nails, and it's area.)
If a discovery will be made for any infection or allergic reaction during an appointment, the service can not be carried out!
I understand certain medical conditions/treatments can affect nail enhancements (such as diabetes, excessive sweating, radio- or chemotherapy to mention a few).
I agree that the nail technician will put chemicals on my nails during the nail appointment, which is necessary to complete the service.
Not only that, but I understand I can develop allergies to any ingredient at any time.
Use cuticle oil and hand cream between appointments to keep your skin and nails hydrated. Dehydrated nails can curl and detach from the product, causing lifting at the edges.
Using cuticle oil can make your manicure aesthetically better looking, but also longer lasting. You can massage a few drops of cuticle oil into the cuticle area and underneath your nails for optimum results at least twice daily.
Things you can do to care for your nails:
* Wear gloves when cleaning, working with chemicals, washing dishes, gardening, etc.
* Moisturise your hands after washing them
* Use cuticle oil a minimum of twice a day, but there is no limit for usage
* Don't use your nails as tools—don't open cans with them, don't scrape anything with them, don't use them as screwdrivers, etc.
* Your nails are your jewellers, mind them like one.
This form needs to be filled out before each appointment for insurance purposes.
